Visit RFUCAS to complete your application. Applications must be completed by the deadline for the quarter you wish to begin the program.

Education

The following are required for admission into the Certificate in Lifestyle Medicine:

• Bachelor's degree from a regionally accredited college or university. Competitive applicants will have earned an MD, DO, DPM, DNP, or other graduate degree if holding APRN license.
• Cumulative minimum grade point average of 2.75 on a 4.0 point scale in the last two years of study.
• Alternatives to these admission requirements may include relevant academic work or professional work experiences, with preference for candidates with coursework in the life/physical sciences.

Prerequisites

• A four-quarter hour (2.67 semester hours) course each in general biology with lab, general chemistry with lab, and anatomy or physiology. Show evident of a grade of “C” or better on an official transcript.

Standardized Tests

No GRE score is required.

If you do not hold U.S. citizenship or permanent residency, you will need an official score report from TOEFL. (This may be waived if you have been a full-time student at a U.S. college or university for at least two consecutive years, or if you are from a country in which English is a primary language.)

Basic TOEFL Requirements

If you earned a bachelor's degree from outside the U.S. and/or you do not hold. U.S. Citizenship or permanent residency, you are required to demonstrate proof of English proficiency via an official TOEFL report that includes: a total score and category scores in reading, writing, listening and speaking.

Minimum scores:
IBT (Internet-based test) of 100 or the equivalent is recommended for graduate school with no category score being lower than 22.

Please note:

• Test scores must be a test taken within 2 years of the date from when a complete application is submitted.
• The language test score requirement may be waived if you have demonstrated academic success as a full-time student at a U.S. college or university for at least two consecutive years, or if you are from a country in which English is a primary language.

Letters of Recommendation

One letter of recommendation is required, two preferred, from professionals and/or academicians who know you well (i.e. pre-health advisors/committees, professors or supervisors). We encourage letter writers to share their contact information. Letters must include a signature and be on official letterhead. Letters must be submitted through RFUCAS.

Current Resume or Curriculum Vitae

A resume or curriculum vitae is required.

Transcripts

Please submit a transcript for your bachelor's degree. Also, if applicable, please submit a transcript for your professional degree (MD, DO, DPM, DNP, or other graduate degree if holding APRN license) to RFUCAS.

Only official transcripts with degree conferral will be accepted.

If you have studied outside the U.S., you will need to have your transcripts evaluated for U.S. equivalency using a service like www.wes.org or www.ece.org.

Professional Experience

Professional experience in health promotion is desirable.
