Attention: Faculty Performance Evaluation Forms should not be confused with the Effort Reporting Form required of employees within a department who administer sponsored projects and/or conduct sponsored activities.

CHAIR Evaluation

Annual Chair Evaluation

  1. Chair Assessment (doc or pdf) (To be completed by the Department Evaluation Team)
    • The department chair appoints an evaluation team from within the department. The team is composed of one senior and one junior faculty member and will represent the department faculty.
  2. Peer/Self Assessment (doc or pdf) (To be completed by the chair for both the "self" and "peer" review)
    • The department chair is required to complete a self assessment. The peer assessment should be completed at the discretion of the dean or the individual chair.
    • Completed self and peer assessments should be sent to the appropriate dean.
  3. Dean Assessment (doc or pdf) (To be completed by the dean for evaluation of department chairs)
    • The dean is required to evaluate the performance of his/her department chairs.
    • Form will be reviewed/discussed and signed off on by the department chair and dean.

Note: Original Chair Evaluations should be archived by the Office of the Dean.

5 Year Chair Evaluation
