This program is co-sponsored by the Illinois Council of Health-System Pharmacists and Rosalind Franklin University College of Pharmacy. The Illinois Council of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.  This program is equivalent to 2 contact hours or 0.02 CEUs.

Instructions for completing the live program evaluation online and downloading a statement of credit will be provided at the meeting.  Attendees must complete the evaluation online in order to received CPE credit.  To verify attendance, attendees will need to enter a meeting code for each presentation.  The meeting code will be announced at the presentation. Participants must attend the entire session to receive the designated credit - no partial credit will be awarded.
